Full Job Description

We have multiple exciting vacancies for Prescribing and Non-Prescribing Clinical Pharmacists. Successful candidates will be apart of a dynamic , friendly and exciting team . Competitive salary £20 -£30 per hour as self employed ,which is matched to the candidates skills and qualifications, this can be discussed at interview stage. Currently we are looking for pharmacists to work between Monday to Friday . The contract will be self employed.

The post holder will be an experienced clinical pharmacist, who acts within their professional boundaries, to develop, and work within a team of pharmacists. They will work as part of a multi-disciplinary team taking responsibility for key development work covering areas of chronic disease management and undertake clinical medication reviews using ECLIPSE.

Role Requirements (Clinical Pharmacist)

· Applicants MUST have previous general practice experience

·Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council

·Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role

·Qualified or enrolled on CPPE Clinical Pharmacist Training pathway.

·Experience in supporting common acute and long-term conditions

·Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support

·Strong clinical leadership, communication and interpersonal skills

·Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role

Role Responsibilities (Clinical Pharmacist)

·To contribute to improve health outcomes in long term conditions as a member of the multidisciplinary primary care team

·To provide specialist knowledge and advice on pharmaceutical matters, including cost-effective prescribing and clinical guidelines for disease treatment, and to also provide evidence-based education and training for colleagues in partnership with the practice

·To provide structured medication reviews to a high level of standards incorporation with ECLIPSE

·To lead on implementation of key projects.

·To support practice achievement of key Impact and Investment Fund and QOF targets as required

Precision Meds looks to provide clinical services to ICS , PCNs and Surgeries by utilising the ECLIPSE platform ensuring the right patients are being seen by the right person at the right time therefore having impactful interventions.

Requirements

  • Must have an additional screen- a laptop will be provided
  • Good internet connection of at least 100mb
  • ECLIPSE- desirable if not familiar- training will be provided
  • Able to use EMIS
  • Flexible to cover additional hours on weekends if required.
